You need to get in the right state of mind to control your weight and diet. For this you have to set a reasonable goal. Do baby steps. Try for five, then ten, then twenty pounds, but only once you have gotten past the first obstacle. When it come to your exercise routine, you should work out for ten minutes, then 15 minutes, then 20 minutes and so on and until you have reached an hour. This way you can gradually, each week, get yourself back into the game. You can also start the routine off slowly as a warm-up and then push yourself harder every a couple of minutes and then gradually bring yourself back down.
That is one way that will help you break your physiological needs, or you can consult a professional to help you get back on the right track.
